Cannabis use during pregnancy and lactation is increasing in line with the legalisation of this substance for both recreational and therapeutic purposes. This article reviews the pharmacokinetics of tetrahydrocannabinol (THC— cannabis’ component responsible for the “high” effect) in pregnant and breastfeeding women, as well as its potential short- and long-term effects on the child. We also examine the impact of cannabis on lactation and breast milk composition. Furthermore, the paper explores maternal beliefs and motivations for cannabis use, the ethical dilemmas faced by health professionals when balancing the benefits of breastfeeding against the risks of cannabis exposure and provides practical recommendations for lactation consultants. We also discuss cannabidiol (CBD) consumption.
